Introduction
Root Cause Analysis (RCA) is only carried out on important, generally avoidable, infections - so it needs to be done well. There is also an increasing constraint from the Department of Health and others about what an investigation will include. Given our local success in reducing healthcare associated infections, we have to recognise that the RCA process may be new to many participants. This is therefore an attempt to bring all the relevant guidance into one document.
